Mental Health Care Power of Attorney
A durable mental health care power of attorney appoints a person you have chosen and trust to make mental health care decisions for you in the event that you are unable to care for yourself. It is usually the same person named as your agent in a medical power of attorney. If necessary, your agent will make decisions about treatment plans, records disclosure, medication consent, and admission to a behavioral health facility.
Preparing power of attorney documents involves sensitive choices. Talk to your doctor and trusted loved ones or friends to be sure you choose a representative who will truly speak for you. When you are ready to review your current power of attorney documents or if you need to create them, call our office to set up an appointment.
