A durable mental health care power of attorney appoints a person you have chosen and trust to make mental health care decisions for you in the event that you are unable to care for yourself. It is usually the same person named as your agent in a medical power of attorney. If necessary, your agent will make decisions about treatment plans, records disclosure, medication consent, and admission to a behavioral health facility.
